Student-athletes must obtain prior written approval from the Director of Athletics or designee and the Faculty Athletic Representative prior to practicing or competing in an event while not representing the institution during or between terms of the regular academic year, per Big Ten Conference rules (17.02.9). In order to protect the eligibility of all student-athletes, prior written approval must also be completed for any competition not scheduled through the UW or while not representing the UW at any time (i.e. Olympic/International Events, Summer Club Competition).
This form must be received by compliance 48 hours prior to the start of the competition.
If you plan on practicing or competing in an event while not representing the UW during or between terms of the academic year, you must receive prior written approval from the Director of Athletics or your Sport Administrator. Not filling out the form prior to participating in the event is a violation.

For individual sports, anytime a member of a UW team wishes to participate in a competition while not representing the UW, the student-athlete is competing unattached. An unattached competitor cannot:
- Wear/use UW apparel that includes UW identification;
- Receive expenses from the UW; and
- Receive instruction from a UW coach.
For team sports other than basketball, a student-athlete may compete outside of the declared playing and practice season as a member of an outside team in any noncollegiate, amateur competition during any official vacation period at the UW.
In the sport of basketball, a student-athlete may not compete in any organized competition except when representing the UW in intercollegiate competition (except Certified Summer Leagues).
